Decision-Making Template

For a decision that's big enough to deserve more than a gut call. Fill it out as you go, not after you've already decided, the point is to structure the thinking, not justify it after the fact.

[A brief title identifying the decision to be made.]
1. Define the problem
What is the problem, and why does it need to be addressed now? [Description.]
What are the desired outcomes of addressing it? [Objectives.]
2. Gather information
What sources will you consult, reports, feedback, data? [Sources.]
Who needs to be involved in this process? [Stakeholders consulted.]
3. Consider alternatives
[List the possible solutions, brainstormed with the team if this affects them.]
[Pros, cons, and short- and long-term impact of each.]
4. Make the decision
Which option are you proceeding with? [Chosen solution.]
Why was this option selected, what data or insight supports it? [Rationale.]
5. Plan the rollout
[Key steps needed to implement the decision.]
How will this be communicated, and what's the key message? [Communication strategy.]
6. Execute and communicate
[Implementation checklist, with notes on adjustments as they happen.]
[Log of updates communicated, and any reactions or pushback.]
7. Gather feedback and adjust
What feedback came in after implementation? [Feedback received.]
What changed in response, and what would you do differently next time? [Adjustments and lessons learned.]
